About Dr. Likhiteswer Pallagani

Dr. Likhiteswer Pallagani is a specialist urologist with focused training in uro-oncology and robotic surgery, currently practising as Consultant Urologist, Uro-Oncology and Robotic Surgeon at KIMS Institute of Renal Sciences & Advanced Urology, Secunderabad. With 400+ robotic procedures and 1,000+ laparoscopic procedures to his credit, he brings a level of minimally invasive surgical volume unusual for a urologist at his stage of career.

His academic foundation was laid at AIIMS Jodhpur — one of India's most rigorous urology training institutions — where his MCh Urology provided deep exposure to complex oncological, reconstructive, and minimally invasive urological surgery in a high-volume referral setting. Following completion of his MCh, Dr. Pallagani was selected for the Vattikuti Foundation Fellowship in Uro-Oncology and Robotic Surgery — one of the most prestigious fellowships in Indian robotic urology, providing dedicated hands-on robotic surgical training at the Asian Institute of Nephrology and Urology (AINU), Hyderabad, under the Vattikuti Urology Institute's mentorship programme.

This fellowship was followed by specialist practice at Yashoda Hospitals, Somajiguda, where he built his independent robotic surgical practice before joining KIMS. His robotic surgical experience covers the full spectrum of urological oncology — robotic radical prostatectomy, robotic partial and radical nephrectomy, robotic radical cystectomy with intracorporeal diversion, robotic adrenalectomy, and Video Endoscopic Inguinal Lymph Node Dissection (VEIL) for penile cancer.

His endourological practice complements his robotic work: RIRS and URSL for kidney and ureteric stones, PCNL for complex stone disease, and laser enucleation of the prostate (ThuFLEP — Thulium Fibre Laser Enucleation of the Prostate) using the latest laser technology. He has published research in indexed urology and oncology journals and has won best video awards at national urology conferences, including the SUZICON 2022 Best Video Prize.

Areas of clinical expertise

Robotic Uro-Oncology

Dr. Pallagani's primary subspecialty expertise is in robotic-assisted surgery for urological malignancies. He has performed 400+ robotic procedures with specific focus on:

  • Robotic Radical Prostatectomy (RARP) — for prostate cancer. Nerve-sparing technique for preservation of continence and potency in eligible cases. Urethrovesical anastomosis. Extended pelvic lymph node dissection.
  • Robotic Partial Nephrectomy — kidney-sparing surgery for renal tumours. Off-clamp or minimal-clamp technique to preserve kidney function. For tumours up to 7cm in suitable anatomy.
  • Robotic Radical Nephrectomy — for larger or more complex renal tumours. Retroperitoneal or transperitoneal approach.
  • Robotic Radical Cystectomy (RARC) with intracorporeal diversion — for muscle-invasive bladder cancer. Ileal conduit or orthotopic neobladder reconstruction, fully intracorporeal — avoiding an external incision for the diversion.
  • VEIL — Video Endoscopic Inguinal Lymph Node Dissection for penile cancer. Minimally invasive groin node dissection avoiding the wound complications of open inguinal dissection.
  • Robotic Adrenalectomy — for phaeochromocytoma, Cushing's adenoma, Conn's syndrome, and adrenal metastases requiring resection.
  • Robotic Pyeloplasty — for UPJ obstruction. Anderson-Hynes dismembered pyeloplasty with robotic precision.
  • Robotic Renal Transplant — experience in robotic-assisted kidney transplant (RAKT) from AINU training.

Endourology & Stone Surgery

Minimally invasive management of kidney and ureteric stone disease across all modalities:

  • RIRS (Retrograde Intrarenal Surgery) — flexible ureteroscopy with holmium or thulium fibre laser for intrarenal stones
  • PCNL (Percutaneous Nephrolithotomy) — including Mini-PCNL — for large and complex stone disease
  • URSL (Ureteroscopy + Laser Lithotripsy) — for ureteric stones
  • CLT (Cystolithotripsy) — for bladder stones

Laser Prostate Surgery

ThuFLEP (Thulium Fibre Laser Enucleation of the Prostate) and HoLEP (Holmium Laser Enucleation of the Prostate) — the current gold-standard treatments for BPH. Size-independent, no blood transfusion, and day-care in most cases. TURP remains an option for appropriate selected cases.

Laparoscopic Urology

1,000+ laparoscopic procedures including laparoscopic pyeloplasty, laparoscopic ureteroplasty and uretero-ureterostomy, laparoscopic Boari flap and ureteric reimplantation, laparoscopic simple and radical nephrectomy, and laparoscopic reconstructive procedures.
